New Clients
We want you to get the most you can out of your first appointment, so here are a few guidelines that will help to do just that.
Medical Records
It is very important for the doctor to have your pet’s complete medical history. This will allow the doctor to more efficiently make an accurate diagnosis, and prevent unnecessary duplication of medical tests.
A few days before your appointment, please contact your pet’s previous veterinary hospitals and request that they fax us all lab work, radiology/ultrasound reports, and the doctors actual medical record notes (please – copies of invoices aren’t enough) If you have any concerns about how to go about getting the records please call us – we will be glad to help.
Our fax number is 415-721-2420.
Your pet’s diet and current medicines/supplements
Equally important for the doctor is information about your pet’s current diet, medications and supplements. It is a good idea to bring a list of all of them including brand names, and the amount and frequency of administration. Please bring the actual bottles of medicines and supplements with you so we can see the ingredients. In this way we can help to avoid duplication and also prevent drug interactions or herb/drug interactions.
Arrival
Please arrive 15 minutes before your actual appointment time to allow you to fill out your New Client paper work and let us organize your chart so that you can have the full time allotted with the doctor.
Please let us know if there is anything we can do to make your visit with us as comfortable as possible!
